IPL Explained
Intense pulsed light, as the name implies, is a concentrated amount of non-coherent light energy that is produced by a Xenon flash lamp over a broad spectrum of wavelengths from 400 to 1200 nm. Different types of filters can be used to limit the wavelength/spectrum of the intense pulsed light. This light targets a range of structures or chromophores simultaneously. The Intense Pulsed Light system works on the principle of selective thermolysis by targeting a range of structures or chromophores simultaneously. The light energy is converted to heat energy in the targeted structures and these are heated to the point of destruction. Filters are used to cut-off selective light wave emissions. Therefore, the same device can treat various targets by applying different filters. A large spot size means the number of pulses per treatment area can be reduced, resulting in efficient treatment times. IPL treatment is ideal for the treatment of vascular lesions, permanent hair removal/reduction, pigmented lesions and multiple types of dyschromia, skin rejuvenation therapy for wrinkles, lax and ageing skin.
Hair Removal 640/600 nm filter
o Wavelength
is adapted to the melanin content of the target hair follicle and nearby tissues.
o The
intense pulsed light is selectively delivered to the hair follicle without
harming the epidermis.
o The photon energy is converted to heat energy,
which causes the temperature of the hair follicle to rise.
o This heat causes permanent damage to the hair
follicle and the blood vessels supplying the hair follicle.
o This
synergy, specific and exclusive to Intense Pulsed Light, helps to irreversibly
disable most of the cells responsible for hair growing.
Skin Rejuvenation 570 nm filter
o
During this
procedure, an intense beam of light energy is directed into the affected area.
o
The photon energy emitted by the IPL Light penetrates the subcutaneous layers and is absorbed by the
collagen and converted to heat energy.
o
This results in selective thermolysis of the collagen resulting in the
heating and contraction of the collagen bundles.
o
A
photochemical reaction occurs, with the formation of Oxygen Peroxide in the
mitochondria and ROS (Radical Oxygen Singlet) inside and outside the cells.
o
Low amounts
of ROS clean both cells and tissue thus increasing their activity.
o
High amounts
of ROS are toxic and kill the cells (for instance the old fibroblast), thus
stimulating the micro-drainage of the damaged old tissue and the deposition of
new tissue.
o
This causes a biological stimulation, regeneration and realignment of
the collagen and elastin fibres.
o
This stimulated cell turnover helps to generate new
collagen.
o
This helps to reduce the appearance of fine lines and wrinkles, improve
the skin’s elasticity and shrink the pore size.
Acne Treatment 400nm filter
o
This 400nm wavelength has an anti-bacterial effect, thus
destroying the bacteria responsible for acne.
o
The
destruction is not only due to a higher singlet oxygen production but also as a
result of the stimulatory effect of the light on cytokine mechanisms, that stimulate
specific immune-regulatory pathways that play a significant role in
post-treatment healing of inflamed acne skin.
IPL for vascular and
pigmentation
o
The Selected wavelengths when
multi-pulsed are equally suitable for treating pigmented lesions such as age
spots and freckles.
o
Light absorbed by
melanin within the pigmented lesion is transformed into heat energy.
o
This process causes
the breakdown of pigment molecules which are “cleaned up” by phagocytes.
o
They are also shed
off by the normal process of skin renewal.
o
Transient erythema
and darkening of pigment molecules can be expected.
o
These same
specialised wavelengths also help to provide a visible reduction in unsightly
capillaries and diffused redness.

o For Hair Reduction treatments, you will notice a significant reduction in hair growth. Results vary depending on the client.
o For Photo-rejuvenation treatments, the skin will appear radiant, refreshed, with an improved tone and texture. You may see a significant reduction of small red blood vessels and irregular areas of pigmentation may fade. Acne sufferers should achieve fast results and pore size may reduce by up to one third in size.
2. How many treatments are required for Hair Reduction?
o It has been demonstrated that a minimum of 6-8 initial treatments are required depending on the individual.
o Treatments are scheduled 4-6 weeks apart.
o Your specific requirements will be assessed in your consultation.
3. How many treatments are required for Photo-Rejuvenation?
o It has been demonstrated that a minimum of 4 initial treatments are required depending on the individual.
o Treatments are scheduled 3 weeks apart.
o Follow up treatments are recommended every 3-6 months for optimum results.
o More initial treatments may be required to obtain your specific skin goals.
o Your therapist will discuss a suitable program for you.
4. What can I expect after a treatment?
o Directly after a treatment, your skin may look slightly flushed and feel mildly tender.
o This redness and tenderness is common and should settle within a few hours.
5. Is there any After-care treatment advice?
o Wear an SPF30 product and avoid tanning beds, sunbaking and any unnecessary sun exposure for the duration of and for approximately 3 weeks after, completion of your treatment course.
o Avoid using harsh soaps or deodorants on treated skin for 24 hours after a treatment.
o For best results, use only a recommended pre and post professional cosmeceutical skin care range such as Clairderm Cosmeceuticals.
Only professional
personnel with appropriate training should operate an IPL device and conduct
IPL treatments. Unauthorized use or abuse of an IPL in the hands of a novice
may cause thermal injury to oneself or others and may cause irreparable damage
to the individual and device. There are range of specific IPL precautions that
should be noted and an IPL treatment should not be offered to patients who have these conditions. These
are highlighted in the training manual. Make sure that you familiarise yourself
with these. Some general precautions include not offering IPL to patients with the following conditions:
o
Patients with a history
of keloid formation.
o
Photodermatoses and
photosensitivity skin diseases.
o
Patients with a
history of skin infections or Herpes Simplex (cold sores).
o
Skin Inflammation,
lesions, open wounds, infections.
o
Within 6 weeks of
having any type of hair removal treatment.
o
During pregnancy.
o
Patients with skin cancers
or a malignant tumour, immune system defects and Epilepsy.
o
Patients who have
taken Isotretinoin (i.e.Roaccutane) within the last 6 months.
o
Patients with
Cardiovascular disease, Liver disease, Kidney disease, Cancer, Diabetes.
o
Patients taking
anti-inflammatory medication or blood thinners and who have bleeding disorders.
(These have a negative effect on blood clotting and can cause bleeding.)
o
The intake of
Antibiotics or any other medication that can cause photosensitivity as a side
effect – wait 2 weeks before proceeding with IPL
o
Patients with
sun-tanned skin should postpone IPL hair removal by 8 to 12 weeks.
o
A delay is also
recommended in the following cases: Injectables/Fillers: wait 3 to 7 days; facial
peel / microdermabrasion: wait 1 week.
o Assess skin type and skin condition being treated (Refer to doctor if further assessment is required)
o Discuss how treatment works
o Discuss side effects and potential complications and contraindications of the treatment.
o Do not offer IPL to clients or postpone treatment if the patient has any of the conditions highlighted in the precautions section of the device training guide.
o Discuss treatment plan and estimate the number of sessions to achieve the desire results.
o Discuss and agree costs.
o Complete and sign the consent form that is specific to the IPL treatment plan.
o This is a precautionary measure.
o By exposing a small area of skin within the treatment site you will be able to establish potential reactions to the proposed IPL treatment.
o Use the technique highlighted in the training guide.
o Remove make-up and sunscreen.
o Shave hair if treating a hair baring area.
o Choose parameters according to test patches and training manual parameter sheet.
o Use a yellow highlighter to mark grid system on skin.
o Both patient and clinician should apply eye/optical protection before treatment commences.
o Apply a generous amount of cooled conductive gel (non-tinted) to the treatment surface area.
o Commence treatment.
o Monitor treatment and patient reactions at all time and take necessary action where required as highlighted in the training guide.
o Wear sunscreen and minimise sun exposure
o Avoid hot showers and spas for 48 hrs. We recommend Clairderm SPF50+, Broad Spectrum, Moisturising Daily Defence Sunscreen Lotion (CD40503).
o Apply specially designed moisturisers and cleansers to clean skin and use only cold or warm water when using a cleanser.
o We recommend the Clairderm Cosmeceutical skin care range that has been specifically developed for Pre- and Post- treatment care. Cleanse with the Clairderm Gentle Purifying Cleanser. Hydrate and nourish with the Clairderm Nourishing Cream Mask and day/ night creams and serums.
o Avoid the use of make-up for the first 24 hours after treatment. It is recommended that only mineral make-up is used after 24 hours. A doctor should be consulted is the patient develops an infection and cease applying any make-up.
o For axillary hair treatment, it is recommended that deodorants and shaving be avoided for 24-48 hours after treatment.
o Do not pick skin (let pigment slough off naturally)
o Call the clinic/salon if you have any questions or concerns.
o Importantly refer to the training guide for after care treatments that are relevant to specific IPL treatments, make-up and sunscreen.
o The treatment duration of IPL therapy extends for a few months.
o Patients should be reminded of their follow up schedule to maximize the efficacy of the treatment.
o The treatment parameter chart provided should be consulted for the treatment intervals.
Results can be seen after a series of treatments as agreed during the treatment consultation. It is recommended for optimal long-lasting results that the post procedure protocols highlighted in the training manual are adhered to.

IPL treatment addresses a variety of skin concerns and to optimise results, post procedure treatment care is essential. IPL treatments require both general and specific post treatment care. The specific care is highlighted in the training manual. However, the general care is detailed below:
